Depression is  considered "treatment-resistant" if at least four different medications haven't worked and a person has also been seeing a counselor or therapist.

Up until fairly recently, the only option remaining was electroconvulsive therapy

But the FDA has given the nod to TMS (transcranial magnetic stimulation), which not only doesn't need a general anesthetic - you can even read during the treatments.
